(State of Maine will no longer accept the courtesy birth certificate issued by the hospital. Must be on security paper issued by the State where the birth took place. Maybe obtained from the local town or city of birth or the State keeper of vital records.)
5. Copy of social security card
6. Mail in Application
You are not registered for class until Application has been received with deposit.
Registration is on a first come first serve basis.
